SF Discover and short-term custom programs: Students must have health insurance during the entire duration of their program. Students must buy the travel insurance approved by SF State.

SF Discover, STEM Exploration and short-term custom programs health insurance enrollment instructions:

Travel Insurance for Short-Term Programs at San Francisco State University 

San Francisco State University requires that all participants for short-term programs purchase the Patriot travel insurance policy. Please purchase before your program start date.

Patriot Travel Insurance Purchase Instructions:

  1. Go to IMG Patriot PLUS Travel Plan (Brochure)
  2. Enter all the information required:
    1. Length of coverage: from arrival date to departure date
  3. Choose premium amounts:
    1. Deductible = $0
    2. Maximum Limit = $1,000,000
      Please choose the correct policy maximum and deductible. Other amounts do not meet the requirements.
  4. Enter your information:
    1. For the Government Issued ID Number, use your passport number
    2. Residence Address = your address in your home country
    3. Fulfillment and Information Delivery Method = choose the first option (electronic communication – email)
  5. Coverage Options: You can choose NO in both options.
  6. Review medical coverage, personal information, coverage dates, address, and premium. If everything is correct, continue with the application.
  7. Enter Payment Information (credit card and billing address information)
  8. Check the box at the bottom of the page, enter your name in the signature box, and click continue
  9. Complete purchase.
  10. You will receive an email with your insurance card, and other related insurance information.
  11. Upload the purchase proof ‘Declaration of Medical Insurance’ to our departmental secure Box. See the instructions below.
  • Please click on ‘Access plan documents’ in your email after receiving a purchase confirmation email from the insurance company.
